Premier Service is proud to offer a 24-hour emergency assistance program tailored to clients with BI, CAC, CADI, and DD waivers. This program provides continuous support, including on-call counseling and proactive problem-solving for health or personal emergencies unrelated to injury or immediate danger. The service operates seamlessly, offering triage and support to 911/988 assistance, coordination with the support team and other providers, and the nurturing of self-advocacy skills.
Individualized Home Supports (IHS) encompass services tailored for individuals residing in their own or family home. These services offer support and/or training within the community living service categories. Three types of IHS are available: without training, with training, and remote support service delivery. All IHS types can be administered in the individual’s own home, family’s home, or in public community spaces, either in person or remotely.
Support within community living service categories includes cueing, skill maintenance, guidance, instruction, assistance with activities of daily living, assistance with coordination of community living activities, or direct supervision, all of which must align with allowable community living service categories.
Training within community living service categories entails skill-building and instructional services for acquiring, retaining, and enhancing the individual’s community living experience. The training must fall within allowable community living service categories and address identified needs as specified in the individual’s assessment (e.g., MnCHOICES, long-term care consultation).
ICLS services offer assistance and support for older adults who need reminders, cues, intermittent/moderate supervision or physical assistance to remain in their own homes.
ICLS covers assistance and support for eligible people age 65 and older enrolled in the Alternative Care (AC) program or the Elderly Waiver (EW). It includes the following service components:
The Integrated Community Support (ICS) waiver program offers 24-hour awake staff, known as direct support professionals, in our apartments. Rent payments can be fully or partially covered by Social Security Benefits, the MSA Grant, and the MSA Housing Grant. We assist clients in accessing financial aid and managing their benefits. Additionally, rent can be covered in full or in part through gainful employment, a service that Premier Service can support through various programs. All Premier Service ICS program locations provide convenient access to public transit, restaurants, and shopping facilities, many of which are within walking distance.
If you believe that you could benefit from Integrated Community Support, we encourage you to reach out to Premier Service. We would be pleased to engage in discussions regarding qualifications and the range of services available.
Overnight supervision, also known as night supervision, entails the provision of overnight assistance and monitoring by a qualified staff member in the individual’s place of residence. This service is specifically intended for individuals who have been assessed as requiring support in the following areas:
It is important to note that night supervision is not designated as a preventive or assumptive safety service. In other words, it should not be utilized solely for the purpose of monitoring the individual overnight to ensure their safety. Individuals must have a documented need for assistance in one of the aforementioned areas to qualify for this service. Furthermore, it cannot be utilized to remunerate a caregiver who resides in the home as asleep staff. Notably, individuals are not eligible to receive night supervision services for more than 12 hours within a 24-hour period.
