Moving Home Minnesota

The Moving Home Minnesota (MHM) initiative represents a federal demonstration project uniquely crafted to facilitate the transition of individuals from institutionalized settings to independent living within the community. MHM places a strong emphasis on person-centered planning and the delivery of services in the least restrictive and most integrated environments.

Eligibility Criteria

To get MHM services, you must meet certain requirements:

Living in a Qualified Institution: You must be living in a qualified institution for 60 days or more.
Enrolled in Medical Assistance (MA): You must be enrolled in MA before getting services and keep this enrollment while receiving MHM services.
Moving to a Qualified Community Residence: When you leave the institution, you must move to a qualified community residence and still need the same level of care to keep getting MHM community-based services.

Qualified institutions

MHM services are accessible to individuals residing in a variety of accredited institutions, including:

Child and Adolescent Behavioral Health Hospital (CABHH) in Willmar
Community Behavioral Health Hospitals (CBHH)
General hospitals
Institutions for Mental Diseases (IMD), such as Anoka Metro Regional Treatment Center
IMD facilities participating in the 1115 Substance Use Disorder (SUD) System Reform Demonstration
Intermediate Care Facilities for Persons with Developmental Disabilities (ICF/DD)
Nursing facilities
Psychiatric Residential Treatment Facilities (PRTF) for children and adolescents

Qualified Community Residences

To remain eligible for MHM services, individuals are required to transition to a qualified community residence. This may encompass the following:

A residence owned or leased by the member or their family.
An apartment with an individual lease, where the member or their family maintains domain and control.
An assisted-living residence with distinct living, sleeping, bathing, and cooking areas, as well as lockable entrance and exit doors.
A residential home where no more than four unrelated individuals reside.
